  • Original Transfer Certificate (TC)
  • Authorized Vaccination Record (if available)
  • Medical Blood Group Certification Report
  • Disability Certificate Form (if applicable)
  • Parent Profile Identification Proof [Identity Proof Redacted]
  • Tax Identification Card [Financial Identity Redacted]
  • Four (04) Recent Passport-size Photographs
  • Official BPL & Ration Card Records (For Scholarship Students)
  • Certified Death Certificate of Parent(s) (For Orphan Verification)
  • Official Bank Account Statement (For Scholarship Allocation)
